Dr. Cohall grew up in Harlem and is committed to helping vulnerable communities achieve health equity. He trained at both Montefiore and Mt. Sinai Hospitals and is board-certified in Pediatrics and Adolescent Medicine. He is the founder and Principal Investigator for Project STAY (Services to Assist Youth) which has been providing comprehensive, integrated, interdisciplinary services for young people living with, or at-risk for HIV for over 30 years. In addition to the provision of clinical services at New York-Presbyterian Hospital, Project STAY has developed a network of over twenty community-based partners to provide young people throughout NYC with onsite HIV/STI/HepC screening and linkages to PEP/PrEP services at New York-Presbyterian Hospital.

PrEP medicine is covered by many health insurance plans and can only be prescribed by a health care provider. Additionally, there are commercial medical assistance programs that are run by medication manufactures that can provide free PrEP to people with limited income or no insurance coverage. For example, the Truvada for PrEP medication program assists eligible HIV-negative adults in the US who are in need of financial assistance in paying for Truvada for PrEP purposes. These assistance programs can also be used to apply for free PEP services if your insurance does not cover PEP. It is important to note, if you are prescribed PEP after a sexual assault, you may qualify for partial or total reimbursement for medicines and clinical care costs through the Office for Victims of Crimes via the US Department of Justice. Additionally, if you are a healthcare worker who was exposed to HIV on the job, workers compensation could potentially cover your PEP medicine.

HIV stands for human immunodeficiency virus. The abbreviation “HIV” can refer to the virus or to the infection. Unlike some viral infections, the human body cannot fully clear the HIV virus. The HIV virus reduces the number of CD4 cells in the body. These cells are the infection-fighting cells that the human body produces. If left untreated, over time HIV can destroy many of the CD4 cells and lead to an increasing number of other infections, known as opportunistic infections.
Left without antiretroviral therapy, HIV can later progress to AIDS. This is the last and most severe stage of HIV infection that occurs when the body has a weakened immune system and a low CD4 count. A person is considered to have AIDS when CD4 cells fall below 200 cells per cubic millimeter of blood (200cells/mm3). You can also be considered to have AIDS if you develop one or more opportunistic infections regardless of CD4 count. Without treatment people who progress to AIDS typically only survive a few years.

A viral load is the amount of HIV virus in a sample of blood. Laboratories report viral loads as the number of HIV RNA copies per milliliter of blood. HIV attacks the immune system’s infection fighting cells known as CD4. You can tell HIV progression through the CD4 count and viral load blood testing. The loss of CD4 count makes it difficult for a person’s body to fight infection.
The goal of antiretroviral therapy is to suppress a person’s viral load down to an undetectable level and subsequently increase one’s CD4 count. An undetectable level is when HIV in the blood is too low to be detected with a viral load (HIV RNA) test. Even though a viral load may be undetectable, it does not mean that someone is cured of HIV. This is a lifelong virus and some of the virus can still remain inside cells and body tissue.
